Certification of Medical Disqualification
A student-athlete who becomes injured or ill to the point that he or she apparently never again will be able to participate in intercollegiate athletics may receive a medical disqualification, such that they will not count against financial aid limits in future seasons. To document a medical disqualification, the student-athlete, head coach, team trainer, attending physician and a representative from the Compliance Office must sign a form declaring the student-athlete as medically disqualified for subsequent academic years. Further, all supporting documentation must be included with the form.
