Contact and Evaluation Logs
Logs must be kept by coaches recording any and all contacts and evaluations made by the coach. Completed forms must be returned monthly to the Compliance Office. Further, a completed Contact and Evaluation Log must be attached to the Travel Expense Report at the conclusion of each recruiting trip. If a coach makes no contacts or evaluations during the month, it is still necessary to compete and submit a form indicating that none were made.
